1. PURPOSE OF CONSENT

The purpose of this document is to collect the free, informed and voluntary consent of the patient to receive medical care in the field of integrative medicine from a licensed and qualified physician.

Integrative medicine is a healthcare approach that combines conventional medicine with complementary strategies based on scientific evidence, such as nutritional management, lifestyle, emotional support, and, when appropriate, the rational use of nutritional supplements or non-pharmacological therapies.


2. SCOPE AND LIMITS OF THE SERVICE

The attending physician will perform a personalized clinical evaluation and propose an intervention plan tailored to the patient's needs, with the goal of improving their overall health and quality of life.

This approach is medical in nature, although it incorporates complementary tools. Under no circumstances does it replace urgent, hospital, or specialized treatments when necessary, nor does it intend to replace conventional medicine, but rather to integrate with it responsibly.

As with any medical procedure, specific results cannot be guaranteed. Clinical outcome will depend on multiple individual factors, therapeutic adherence, the patient's health status, and their environment.


3. RESPONSIBILITIES AND COMMITMENTS

The patient declares that he/she has truthfully and completely informed the patient about his/her health status, medical history, current treatments, and any other clinical data relevant to the proper care provided by the professional.

He also declares that he has understood that:

  • Treatment will be guided by a licensed physician within the legal and ethical framework of healthcare practice in Spain.

  • Any intervention will be carried out based on current medical knowledge and in coordination, if necessary, with other health professionals.

  • The patient is responsible for following the guidelines, reporting any adverse effects, and communicating any relevant changes in their health status.

  • The professional will act under the principles of Lex Artis Médica , assuming the responsibilities inherent to medical practice in the individual clinical context, but without this implying a guarantee of concrete results.


4. CONSENT

I declare that I have understood the information received regarding the integrative medicine approach and I give my free, informed, and voluntary consent to begin the medical care process with the appropriate professional.
